A 56-year-old gentleman attends the Emergency Department complaining of chest pain. He also reports general symptoms of tiredness, fever and weight loss. He has recently been diagnosed with chronic rhinosinusitis. On examination, his nose is saddle-shaped. ECG was performed which showed widespread ST elevation.
